The UHV COVID-19 Operations Center support university students, staff and faculty by advising, referring, monitoring, communicating, documenting, tracking and educating the campus community about COVID-19. It is headed up by Tara Vaughn, a UHV clinical assistant professor of nursing.
The center conducts contact tracing on both the UHV campus in Victoria and at UHV Katy. Contact tracing is a key strategy to prevent the further spread of COVID-19. It is a method used to identify cases and contacts to interrupt disease transmission with the goal of preventing an outbreak. UHV contact tracers are required to adhere to privacy guidelines.
Below are links to forms that should be filled out by members of the UHV community if they are diagnosed with or exhibit COVID-19 symptoms, may have been exposed to someone with a COVID-19 diagnosis or international travel, or need to return to campus following self-isolation or quarantine.
- Diagnosis or Symptoms Reporting Form
- Potential COVID-19 Exposure Reporting Form
- Request to Return to Campus Form
Once submitted, the forms go directly to the COVID-19 Operations Center, and the person who submitted the form immediately receives an email sent to his UHV email account with further instructions. Employees also should contact their supervisor. Based on the answers provided, the employee or student will receive a phone call from a UHV contact tracer.
