Coronavirus alert update
Dear UHV Faculty, Staff and Students,
If you have not done so already, please be sure to take a look at the university’s Coronavirus information on the UHV website. You also can get to this from the blue Coronavirus information link at the top of the UHV homepage. Much of the information was referenced in UH System Chancellor Renu Khator’s Wednesday email.
The UH System Exposure Prevention page includes detailed information about the 14-day self-quarantine restriction on UH System students, faculty and staff returning from travel, whether personal or university-related, to all countries for which the CDC has issued a Level 2 or 3 Travel Warning related to COVID-19.
The page also has information about the UHV self-reporting form to be filled out by students, faculty and staff who have traveled to a CDC Level 2 or 3 country since Jan. 1, 2020, and those who plan future travel anywhere internationally. When you fill out that form, an email confirmation with the information will be sent to you and to Jay Lambert, UHV vice president for Student Affairs.
Finally, if you have already filled out the UHV self-reporting form, please fill it out again or send the email confirmation that you received to firstname.lastname@example.org. There were some issues with the form that were recently discovered.
