Service Project Form

Remember, you must complete two community projects per semester. One of these projects must be a group project. Good Luck!

Every member must turn in the service project form even for a group project. The chairperson of the group project must also fill out the Group Service Project form for Gamma records.

The Service Project form must have your signature. Print, sign, and mail your form to:

OR print, sign, scan, and email your form to the GBP Secretary.

Individual Service Credit is awarded for volunteerism with any non-profit agency.
Group Service Credit is awarded for volunteerism within a GBP committee and/or special project.
Unless otherwise specified, no one receives credit without a signed service project form.

Group Service Project Form for Chairperson

The Chairperson of any group service project must also fill out this group service project form after the service has been completed. This form will account for all who participated in the group project.
