UHV ESA Membership Requirements
- Membership Fee: $15.00
- Must obtain a student membership to a professional organization such as
- Enrollment in the Teacher Education Program at UHV, Enrollment at a community college, or employment as a teacher and UHV alumni status

Fill out this form and print it or go by Dr. Riedesel’s office (room UW265) to pick up a hard copy of this form. Return the completed form along with the Membership Fee to the Office of Student Activities and Services, room UW133.
