Second Degree BSN Application Checklist

Application Process:

Checkbox Complete the NursingCAS online application for undergraduate admission at the following website https://portal.nursingcas.org/. NursingCAS is the first national centralized application service for students applying to registered nursing programs.

Please note that it takes 4-6 weeks for NursingCAS to process your application. You may start your NursingCAS application May 1, 2012.

Request all official transcripts be sent directly to NursingCAS. To be eligible for this program you must have a minimum 3.0 GPA in your cumulative (all) course work, and a minimum 3.0 GPA in your cumulative (all) natural and social sciences course work. All prerequisite course work must be completed prior to the October 1st deadline. You may obtain a list of the required prerequisite course work for the School of Nursing website at http://www.uhv.edu/nursing/2dbsn/program_requirements.asp.

Checkbox Schedule a time and complete the TEAS V nursing entrance exam. You can register for the exam at www.atitesting.com. Once complete, it is the applicant’s responsibility to send a hard copy of the completed TEAS V scores directly to UHV at nursing@uhv.edu or fax to 361-580-5596.

Please aim for a composite score of 75% or higher and a 75% or higher for the Reading section. You may take the TEAS V exam twice and we will choose the highest score.

Checkbox Please complete the social security form and e-mail the completed form to University of Houston-Victoria School of Nursing at nursing@uhv.edu or fax to 361-580-5596. Click on the link below to access the form http://www.uhv.edu/nursing/Resources/Release_of_SSN_Form.pdf.

Checkbox Submit a copy of your current CPR verification to University of Houston-Victoria School of Nursing at nursing@uhv.edu. Hospital requirements for clinical rotations require that you be CPR certified. Only American Heart Association (AHA) and American Red Cross (ARC) Health Care Provider CPR verification will be accepted. You must take the healthcare provider class for BLS (CPR and AED) program. We do not accept any online CPR certifications!

Submit records of the following immunizations to University of Houston-Victoria School of Nursing at nursing@uhv.edu.

Checkbox Completion of Texas minimum State vaccine immunization requirements.

a. Two-step TB skin test within one year

b. Tetanus-diptheria toxoid (Td) within the last 10 years

c. Measles/Mumps/Rubella vaccinations(s)

d. Seasonal Influenza

e. Varicella vaccination(s)*
    Varicella immunity must be confirmed by one of three ways:
         1. a signed provider note verifying that the student was diagnosed with Varicella
         2. a blood titer indicating immunity
         3. verification of receipt of the Varicella immunization

f. Three-injection hepatitis B vaccination series or serologic confirmation of immunity to hepatitis B virus.

Please note: The students who are invited to the interview will be notified by the end of October.

Acceptance:
All students, who are invited for acceptance into the UHV School of Nursing Second Degree BSN program, must pass a criminal background check and drug screen prior to final admission in January.
