Second Degree BSN Application Checklist
Application Process:
¨ Complete the NursingCAS online application for undergraduate admission at the following website https://portal.nursingcas.org/. NursingCAS is the first national centralized application service for students applying to registered nursing programs.

Request all official transcripts be sent directly to NursingCAS. To be eligible for this program you must have a minimum 3.0 GPA in your cumulative (all) course work, and a minimum 3.0 GPA in your cumulative (all) natural and social sciences course work. All prerequisite course work must be completed prior to the October 1st deadline. You may obtain a list of the required prerequisite course work for the School of Nursing website at http://www.uhv.edu/nursing/2dbsn/program_requirements.asp.
¨ Schedule a time and complete the TEAS V nursing entrance exam. You can register for the exam at www.atitesting.com. Once complete, it is the applicant’s responsibility to send a hard copy of the completed TEAS V scores directly to UHV at nursing@uhv.edu or fax to 361-580-5596.
¨ Please complete the social security form and e-mail the completed form to University of Houston-Victoria School of Nursing at nursing@uhv.edu or fax to 361-580-5596. Click on the link below to access the form

Please Note:
The NursingCAS application, completion of all program prerequisites, submission of all official transcripts, copy of TEAS V scores must be received and the NursingCAS application verified by the October 1, 2011 deadline. The Social security form is due to the UHV School of Nursing office prior to the
October 1, 2011 deadline.
Interview
All students, who are invited to the interview, must provide the following criteria for acceptance in the interview:
¨ Submit a copy of your current CPR verification to University of Houston-Victoria School of Nursing at nursing@uhv.edu. Hospital requirements for clinical rotations require that you be CPR certified. Only American Heart Association (AHA) and American Red Cross (ARC) Health Care Provider CPR verification will be accepted.
¨ Submit records of the following immunizations to University of Houston-Victoria School of Nursing at nursing@uhv.edu.
Completion of Texas minimum State vaccine immunization requirements.
a. Two-step TB skin test within one year
b. Tetanus-diptheria toxoid (Td) within the last 10 years
c. Measles/Mumps/Rubella vaccinations(s)
d. Seasonal Influenza
e. Varicella vaccination(s)*
Varicella immunity must be confirmed by one of three ways: a) a signed provider note verifying that the student was diagnosed with Varicella b) a blood titer indicating immunity c) verification of receipt of the Varicella immunization.
f. Three-injection hepatitis B vaccination series or serologic confirmation of immunity to hepatitis B virus.
Please note: The students who are invited to the interview will be notified by the end of October.
Acceptance
All students, who are invited for acceptance into the UHV School of Nursing Second Degree BSN program, must pass a criminal background check and drug screen prior to final admission in January. Please note: The students who are invited for acceptance into the program will be notified by the end of November.
