The meeting was called
to order at 2:35 p.m. by Genaro Cortez, Safety and Risk Manager.
1. The minutes of the May 18, 2006 meeting were reviewed and approved as written.
2.
Unfinished Business
a. Automated External Defibrillator (AED) Application Status
·
One AED unit has been received. It is yet to be determined where the unit
will be installed.
·
Unit will be placed in strategic location based on State mandated
criteria.
·
Training will be a crucial step. 4-6 people will be trained per AED unit.
Safety and Risk Manager has spoken with Ricky De La Garza of the Victoria Fire
Department EMS and he has agreed to do initial and annual AED training.
·
It is the responsibility of the UHV Safety Office to have a medical
director involved in the AED program. The Safety and Risk Manager will contact
the local medical director.
·
The unit is very compact and has an alarm that sounds when the door is
open to alert others in the area of an emergency.
·
There will be a similar program to apply for AED units in the next fiscal
year. The Safety and Risk Manager will apply for additional AED units.
·
The Business
Contingency Plan review is complete.
·
The final plan
and appendices was 142 pages in length including a new Section F entitled
“Hurricane Plan”.
·
Hard copies have
been distributed to the Emergency Response Team and President’s Cabinet
members. Additional copies will need to be made by requestor.
·
UHSSL received
their copies and Linda Manzano will check with UHSCR to make sure they received
their copy. A notice will be sent to inform Faculty, Staff, and students that the
Business Contingency Plan is available on the web.
3. New
Business
a. FY06 Safety, Health, Risk Management Goals and Objectives
·
The Safety and
Risk Manager is working on FY07 Safety, Health, Risk Management Goals and
Objectives and will send a draft to the committee.
·
Comments and
feedback should be sent to the Safety and Risk Manager.
b. Emergency Evacuation procedure review
·
The Emergency
Evacuation Procedure review is currently in progress.
·
The day and night
evacuation procedures have been updated.
·
Safety and Risk
Manager is currently working on updating all the evacuation rosters.
·
Next step of
review will be making procedure assignments that will have number of people that
have assignments but needs to be updated due to organizations changes.
·
Goal is to
complete update before fall semester starts.
·
It is recommended
that these procedures be reviewed annually
by UHV, UHSSL, and UHSCR.
· The UHV Campus Safety Office compiles and published this annual crime statistics and security report by October 1 of each year.
·
The Campus Safety & Risk Manager compiles
crime statistics from the Victoria Police Department for the
· Also, reportable incidents in the Safety Office incident log are included when they are known to have not been included in the police statistics.
d. SORM200 Report
· The Safety & Risk Manager is currently working on the annual SORM200 report for the State Office of Risk Management.
· State agencies, courts, and institutions of higher education are reminded that the annual reporting of risk management expenditures, insurance purchases, and non-workers' compensation losses will be due by October 30, 2006 via the on-line reporting system.
· All information to be reported will be for the period beginning September 1, 2005, and ending August 31, 2006. Agencies are encouraged to begin reviewing the data to be reported.
e. Safety & Risk Advisory Committee Membership

This committee is
formed on a voluntary basis and reviewed annually. If a member can not continue to serve on the
committee member please notify Safety and Risk Manager with your
replacement. If you want to stay on
committee no notification is necessary.

Regional Open Enrollment Training session to be hosted by the
o A tentative agenda was provided by SORM. Two courses, Ergonomics Assessment Training and 15 Passenger Van Safety, were included per request.

It was reported by the Safety and Risk Manager that

o The training will take place in Multi-Purpose Room and one of three days may also include the Alcorn Auditorium. Volunteers will be needed during event to help participants register and locate rooms. SORM will take care of registration thru their Web site.
o Judy Lucas inquired about the Blood borne Pathogens course – the course will be offered as a refresher course and should benefit Facilities Services personnel and Biology Lab Faculty, staff, and students.

o The monthly fire extinguisher inspection was completed by Pete.
o Pete met with the IT department and discussed the issue of the Dell batteries catching fire and informed them of which fire extinguisher to use if this should happen.
o The Shelter-in-Place kits were checked and are good for another 6 months.
o A CO2 fire extinguisher has been ordered for the UHSSL Biology Lab.

h. Worker’s Compensation Report of Employee Injuries - Payroll Benefit Manager
· Denise Prescott reported 1 claim filed in May, 2 in June and 0 in July. For the fiscal year to date (9/1/05 – 7/31/06) 7 claims have been filed with 1 being denied.
i. Incident Report - Quarterly Summary - Safety and Risk Manager
·
Safety and Risk
Manager reported a heat exhaustion incident. A UHV employee working
commencement felt faint. There was another incident of an employee fainting and
transported to the hospital but not sure if it was heat related. Safety and
Risk Manager cautioned everyone on the importance of drinking plenty of fluids
and taking necessary breaks while exposed to the heat. Training &
Continuing Education Coordinator, Kori Wilson, will look into offering heat
stress education training.
