State law requires that you be informed of the following:

  • with few exceptions, you are entitled on request to be informed about the information the University collects about you by use of these forms;
  • under sections 552.021 and 552.023 of the Government Code, you are entitled to receive and review the information;
  • under section 559.004 of the Government Code, you are entitled to have the University correct information about you that is incorrect.

Authorization of Payment Form (Prior Semester/Prior Year Balance) - Use this form to request approval from the Office of Financial Aid to use your current semester financial aid to pay for a prior semester or prior academic year past due balance.

Chapter 33 Tuition Agreement Form - Use this form to have your Post 9/11 GI Bill credit applied to your student account.

Competitive Scholarship Waiver Form - Use this form to request that the Competitive Scholarship Waiver be applied to your student account if you meet the requirements to receive this waiver.

Hazlewood Exemption Application For Dependents - The HE-D application is for eligible children and spouses of veterans who wish to apply for the Hazlewood Exemption or for children to whom hours have been transferred through the Legacy Program.

Hazlewood Exemption Application For Veterans - The HE-V application is for veterans who wish to apply for Hazlewood benefits as new or previous recipients OR to request their unused hours be transferred to an eligible child.

Jaguar Card Agreement Form - Use this form to request a replacement jaguar card for a lost, stolen, or not received jaguar card.

Peace Officer Exemption Form - Use this form to request that the Peace Officer Exemption be applied to your student account for your criminal justices courses for a particular semester if you meet the requirements to receive this exemption.

Texas Tomorrow Request Form - Use this form to request Texas Tomorrow Funds be applied to your student account for a particular semester.

Tuition and Refund Appeal Form - Tuition Refund for Medical Emergency/National or Natural Emergency/Military Duty/University Error/Extreme Hardship - UHV will consider tuition refund exceptions to state guidelines in the cases of documentable medical emergencies, national or natural emergencies, military duty, university error, or extreme hardship.
